This position is responsible for the day-to-day support of assigned individuals. Works in cooperation with Residential Support Coordinator (RSC), Residential Support Manager/RSM, Community RN, Consultant Dietitian, Consultant Occupational Therapist, and other support services staff, as applicable. The PSA will assist in advocating for the removal of architectural, environmental, attitudinal, financial, employment, communication, transportation, community integration, technology and any other barriers to the persons receiving services through TNC Community. The PSA will report all suspected/alleged abuse and neglect in accordance with the TNC Community policies.


Must be at least 18 years of age.
Must possess a High School Diploma or GED.
Must participate in all mandatory training and yearly re-certifications, as required.

The position requires sufficient communication skills: written, verbal, and nonverbal, to effectively engage with individuals, staff and administration.

Ability to clear background checks and drug/alcohol screenings as required.
Current CPR/First Aid certification.
Maintain current Abuse/Neglect Training.
Completion of Gentle Teaching or Equivalent.
Must participate in all mandatory trainings and re-certifications as required.
Ability to drive motorized vehicles, when applicable for position.
Valid class E Missouri driver’s license or equivalent, as required for position.
NOTE: Specific assignments may not require a driver’s license.
Must have own transportation and proof of insurance, when applicable.
Ability to lift 75 pounds (weight may vary).
Flexible schedule.
Ability and competency to utilize a computer and needed software applications.



1. Communicates well with individuals and their friends, families, and guardians; as well as supervisors, co-workers, and other stakeholders in accordance with TNC Community policy and procedures.
3. Shows concern for individuals and maintains and protects their human rights and privacy.  Interacts with and responds to the needs of individuals.
4. Is informed and educated regarding each supported person’s Individual Support Plan (ISP) and is able to discuss the content such as medication needs, dietary needs and other information within that plan.
5. Following directions of the Individual Support Plan, supports individual to focus on specific identified ISP outcomes and goal activities.  Documents information regarding success level achieved for each activity and response of individuals during the activity.
6. Participates in planning meetings for/with individuals when invited.
7. Documents daily observations and routine supports per waiver guidelines before leaving duty each day. Responsible to double check documentation on all individuals receiving services prior to leaving shift.  Documentation may include, but is not limited to, daily progress/T-log notes, intake & output, personal hygiene, seizures, medications, etc. to include submitting all necessary reports.
8. When Event Reports/Medication Error reports are required, employee completes the report prior to leaving the service area the same day the event is identified.
9. Assists individuals to achieve appropriate, comfortable, personal appearance throughout the day, and supports with necessary adjustments as needed.
10. Supports individuals, as appropriate, to maintain personal belongings and bedrooms as well as common areas of the home or specified service locations to ensure a clean and healthy environment.
11. Works as a “team” with other assigned staff to provide optimum supportive environment for individuals and to complete identified tasks.

2. Observes for and responds to signs/symptoms of health issues related to the individual(s) in a timely manner and reports same to Health Support Coordinator or designated staff member. (In cases of life-threatening emergencies, 9-1-1 would be called first).
3. When applicable, is responsible for notifying Community RN On-Call by telephone of any identified medication/treatment errors that occur on the same day that they occur. This allows the Community RN On-Call to notify the physician and potentially correct for the error.
4. When applicable, administers and documents, as legally required, all medication(s) and treatment(s) prescribed for individuals.
5. Supports assigned individuals by assisting with meal time, hygiene needs and personal care, dressing, medication administration, leisure/recreational activities, and all other areas of life, utilizing adaptive equipment as trained when appropriate.
6. Reports safety, maintenance, and/or adaptive equipment needs promptly.
7. Utilizes appropriate transfer techniques, positioning, and safety activities as needed and/or ordered. Lifting of individuals will always be completed with two (2) personnel working together, where applicable.
8. Adheres to dietary guidelines concerning content and preparation of all meals.
9. Reads “high” designated T-logs before each shift and “communication book,” previous progress notes, bowel records, S-Comms, etc. as applicable at assigned service area at beginning of each work shift. Enters applicable information into same as needed.
10. Is familiar with TNC Community emergency procedures. Participates in training and drills.
11. Maintains required staffing ratio at all times.

Is responsible for being aware of requirements to maintain certification(s) and other training and/or health needs required to maintain employment with TNC Community.  Will attend training offered by TNC Community at no cost in order to maintain employment status or arrange to obtain training elsewhere at own expense. NOTE:  PSA must complete a successful introductory period to receive training offered by TNC Community at no cost.
2. Times in and out correctly and gains approval of and notifies direct supervisor of variances in routine schedule.
3. Arrives at assigned site on-time for shift, dressed appropriately per policy and procedure, and prepared to provide support for assigned individuals.
4. Provides accurate contact information including immediate notification of any change in contact information by completing a PAF and submitting to his/her direct supervisor.


May provide shopping support for individuals when assigned. Is responsible to obtain separate receipts for purchases of individuals and turn all receipts in to supervisor immediately on return or as designated.


1. Maintains TNC Community vehicles in an organized, clean, safe, manner, as applicable.
2. Drivers will ensure safe and continuous operation of TNC Community vehicles by reporting repair needs, accidents, and traffic violations immediately.
3. Monitor facilities for cleanliness, safety issues and functionality for the individuals supported.

2. Complete other related duties as assigned.
